Anaemia (Low Haemoglobin)

Anaemia means low haemoglobin. It is extremely common in India, especially in women and children – but it is a sign, not a diagnosis. The cause must be found.

Symptoms

Tiredness, breathlessness on walking, palpitations, dizziness, pale skin, poor concentration, brittle nails, craving to eat mud or ice.

Common causes

  • Iron deficiency (poor diet, heavy periods, worms, bleeding from the stomach or bowel)
  • Vitamin B12 or folate deficiency (common in vegetarians)
  • Thalassaemia trait or sickle cell disease
  • Kidney disease, chronic infection or inflammation
  • Bone marrow problems – aplastic anaemia, MDS, leukaemia

When to see a haematologist

  • Haemoglobin not improving after 2–3 months of iron
  • Low white cells or platelets along with low haemoglobin
  • Repeated blood transfusions needed
  • Anaemia with weight loss, fever or bleeding

Treatment

Treatment of the cause; oral or intravenous iron infusion (a single day-care visit in many cases); B12 injections; and specialist care for marrow disorders.
