Enlarged Liver and Spleen (Hepatosplenomegaly)

An enlarged spleen (splenomegaly), with or without an enlarged liver, is often found on examination or ultrasound. It always needs an explanation.

Symptoms

Fullness or heaviness on the left side of the abdomen, early satiety, or symptoms of the underlying disease (anaemia, fever, low platelets).

Common causes

  • Infections – malaria, kala-azar, typhoid, tuberculosis
  • Liver disease with portal hypertension
  • Haemolytic anaemias – thalassaemia, sickle cell disease, hereditary spherocytosis
  • Blood cancers – CML, myelofibrosis, lymphoma, leukaemia
  • Storage disorders (such as Gaucher disease), especially in children

Evaluation

Blood counts and smear, liver tests, ultrasound/Doppler, infection tests, and bone marrow examination or molecular tests (BCR-ABL, JAK2) when a blood disorder is suspected.
