Low Platelets (Thrombocytopenia)

Platelets help blood to clot. A normal count is 1.5 to 4.5 lakh per microlitre. A low count is called thrombocytopenia.

Symptoms

  • Pin-point red spots (petechiae) or bruises on the skin
  • Bleeding gums, nose bleeds, blood in urine or stools
  • Heavy menstrual bleeding
  • Many people have no symptoms and the low count is found on a routine test

Common causes

  • Infections – dengue, malaria, viral fevers
  • Immune thrombocytopenia (ITP) – the body destroys its own platelets
  • Medicines, alcohol, liver disease with a large spleen
  • Pregnancy-related causes
  • Bone marrow disorders – aplastic anaemia, leukaemia, MDS
  • "Pseudo-thrombocytopenia" – a lab artefact; a repeat sample in a citrate tube may be normal

Treatment

Depends on the cause and the count. ITP may need steroids, IVIG, or newer tablets (eltrombopag); many people with mild ITP need only monitoring. Platelet transfusion is reserved for bleeding or very low counts.
